Your First Visit

In your initial appointment and consultation, the surgeon will review your medical history, gather information regarding your reason for visit and symptoms, explain your diagnosis and propose a treatment plan. You will be given an opportunity to ask questions.

An initial consultation is mandatory for patients undergoing surgery. Often, surgery can be performed the same day as your consultation. However, if a more complex medical history or extensive surgical treatment is required, you’ll be advised to book an additional appointment for your procedure.

Please have nothing to eat or drink after midnight the evening prior to your surgery. You will also need a responsible adult to drive you home and remain with you  post-procedure. Please inform your ride that they will be required to remain in the office during your treatment time which is approximately 3 hours.

We ask that you provide the following information at the time of your initial consultation:

  • Photo identification.

  • Your provincial health care card.

  • A list of current medical conditions as well as your medical history.

  • A list of medications and dosages that you are currently taking.

  • Your extended health insurance card (ie Blue Cross, Sunlife).

  • Your Alberta Works, Veterans Affairs, Alberta Child Health Benefit, NIHB or other coverage cards.

IMPORTANT: If the patient is a minor, we request that the individual’s parent or legal guardian be present please.

Insurance & Payments

At Groat Oral & Maxillofacial Surgery we make every effort to provide you with excellent surgical care with the most convenient financial options. We work closely with you to maximize your insurance reimbursement for covered procedures. Our office will accept preferred benefits from your dental plan when possible. This will require preauthorization of your proposed treatment with your insurance plan(s) in advance of your surgical procedure.

Please be aware that there are some surgical procedures that are not applicable under direct billing to your insurance plan. There are select insurance plans that do not allow assignment of benefits directly to the office providing oral surgical care. Please review your insurance plan prior to booking your surgical procedure.

Every effort will be made to pre-authorize your insurance prior to your appointment. However, there are a number of factors which impact our ability to do so. Each insurer varies in their length of response time and whether they will communicate directly with our office regarding covered amounts or whether they will provide the information to the patient. We invite you to contact our office with any questions regarding your coverage. Our team is experienced in health insurance claims and would be happy to help provide information to help clarify the payment and coverage process.

If you have any questions, contact our office.
